There may well be underlying cultural, physiological or genetic reasons as well but it'll be years until the data is analysed and written up. Its interesting that as yet this hasn't gone off in places like Nairobi, Delhi, Cairo etc where the population density is staggering and the healthcare is almost zero of the vast majority.
